providing the best oral health care to all children and adolescents in a happy and comfortable environment. You will soon

One of our primary goals at an initial visit is to establish a Dental Home for your child through a trusting and positive
relationship. The Dental Home is a concept which is endorsed by the Canadian Dental Association (CDA) and the
Canadian Pediatric Society (CPS). This concept embraces prevention. Children who have a dental home are more likely to
receive preventive oral health care, thereby reducing their risk of developing oral disease.
At the First Visit we will review and discuss your child's health history, provide a complete oral exam, and assess their
risk for developing any future conditions. Only the necessary radiographs (X-rays) will be taken to provide us with a
complete evaluation of your child's oral health status. A thorough cleaning of the teeth and fluoride application may also
be performed.
To help us make your experience as smooth as possible, please bring the following signed and completed documents to
the First Visit:
1) Patient Information / Medical History Form
2) Privacy Acknowledgement Form
3) Financial Policy
